RN - Hospice

Day shift, 8 hours (08:00 - 16:30). This position does not accommodate 4x10 or 3x12 schedules.

The Hospice RN is responsible for providing skilled nursing care to patients in their home environment, ensuring comfort and quality of life for patients facing life-limiting illnesses.

• Conduct patient assessments and develop care plans.
• Provide direct nursing care and comfort measures as needed.
• Educate patients and families on end-of-life care and available resources.
• Manage pain and symptoms using appropriate interventions.
• Collaborate with an interdisciplinary team to coordinate patient care.

• Minimum of 1 year of skilled hospice experience required.
• CPR certification (BCLS) required.
• HOPE documentation experience required.
• Must provide own transportation and have experience with relevant charting systems.
