Schedule:

8-hour day shifts, Monday through Friday. Up to 6 patient visits per day with 30 productivity points standard. No weekend or on-call requirements.
Highlights:
One-on-one patient care in a home health setting with manageable caseloads
EPIC and Oasis charting system utilized for documentation
Clients are strategically grouped to minimize drive time; mileage reimbursed at IRS standard rate
No floating required between units; focused home health role
Free parking and any color scrubs accepted
Local candidates welcome; pending licenses accepted
Requirements:
Active Oregon RN license (pending licenses considered)
Minimum 1 year of skilled home health experience required
Current BLS certification
Oasis experience strongly preferred
Proficiency in admission assessments, start of care, discharge planning, recertification, routine visits, and wound care

Experience with medication administration and management, pain assessment, infection control precautions, and case management
Reliable personal transportation required
COVID-19 primary vaccination series plus one booster required
