• Monday - Friday 8 am - 5 pm with call coverage
• Up to 15 face-to-face patients per month
• Average 4 weeknights call and 1 weekend per month
• Guaranteed 8 hours per day
• Telehealth visits available for system management support
• Primary responsibility for symptom management
• Admissions and recertification face-to-face visits required
• Ongoing assignment with coverage potential
