The Person al Health Navigator serves as an extended care team member who supports high‑risk, high‑barrier patients in navigating the healthcare system to follow through on co‑designed care plans. The role is critical in reducing no‑show rates, closing referral loops, enhancing communication between patients and providers, and improving overall health outcomes. The navigator works closely with the care team to assess barriers, empower patients, and support both virtual and in‑person visits.
Responsibilities- Collaborates with health care teams to identify patients in need of outreach and health care navigation.
- Assist patients in completing referrals, following up on care plans, and accessing specialty care.
- Conducts outreach via phone, virtual visits, and in‑person home or field‑based visits to build trust, assess environmental and social needs, and support care coordination.
- Identifies and addresses barriers to care; supports patients in developing health system navigation skills.
- Advocates for patients and builds relationships with specialty clinics and community partners.
- Provides culturally responsive health navigation education to patients and families on the importance of following through with care plans and referrals. Identifies fears, barriers, and hesitancy, and collaborates with the care team and patients to address them.
- Communicates with care team, specialty care provider, and health plan provider as needed.
- Participates in monthly care conferences to share updates on patient engagement and progress in navigation, and to collaboratively address identified barriers.
- Applies teach‑back and culturally responsive communication tools (e.g., Ask Me 5).
- Tracks and reports on referrals, patient impact stories and other PHN data points as assigned.
- Other duties as assigned.
- Minimum:
High School Diploma or GED. - Achieves Community Health Worker certification requirements within 6 months of hire and maintains certification throughout employment.
- BLS Health Care Provider (First Aid/CPR).
- Valid Oregon driver’s license, sound running vehicle, and maintenance of vehicle insurance required.
- Excellent interpersonal communication and problem‑solving skills.
- Skills to intervene and promote reconciliation, compromise, and positive outcomes in difficult interactions.
- Skilled at using electronic health records as applicable to area of work (EPIC, Wisdom).
- Familiarity with the healthcare system and experience supporting vulnerable or high‑barrier populations.
- Bilingual in English and Spanish.
- Degree in healthcare, social services, or related field.
- Minimum of 1–2 years of experience in a health care, case management, outreach, or community‑based role.
- Basic knowledge of adult learning models.
