LocationBirch Grove Health Center, Southern Oregon (Rogue Valley, between the Cascade and Siskiyou mountains).
Job DetailsPosition supports integrated primary care for patients with substance use disorders and/or severe and persistent mental illness. The team operates in a harm‑reduction philosophy and partners with addiction recovery, behavioral health specialists, local hospitals, and the community justice system.
Responsibilities- Provide comprehensive primary care and psychiatric evaluation for patients with substance use disorders and mental illness.
- Collaborate with the integrated care team, including addiction specialists, behavioral health professionals, and community partners.
- Participate in case management and develop tools and support to help patients achieve personal goals.
- Optional on‑call rotation participation with additional compensation.
- Assist in case management and wrap‑around care coordination with local hospitals and the justice system.
- Four‑day work week for full‑time employees; four weeks paid vacation per year and a week of CME.
- Optional on‑call rotation compensation.
- National Health Service Corps loan repayment site with a medical HPSA score of 13.
- Comprehensive benefits package for providers.
- Graduate from an accredited school with board certification or board eligibility.
- Experience with low‑income children and families and in migrant and community health.
- Fluency in English required;
Spanish language proficiency valued. - Active Oregon license.
- CPR certification and active DEA license.
- Experience treating Substance Use Disorders.
- Experience treating Severe and Persistent Mental Illness.
- Experience giving Gender‑Affirming Care.
