Job Responsibilities

• Provide hospice care through a mix of telehealth and onsite services
• Respond to night and weekend calls, manage admissions, and consult on complex patient cases
• Collaborate with NPs during overlapping call shifts
• Participate in interdisciplinary team meetings and consultations

Qualifications

• Active Oregon license and DEA
• Board Certified or Board Eligible in Family Medicine or Internal Medicine, with ample hospice experience
• BLS certification required

Working Hours

• Monday-Friday, 8:00 AM - 5:00 PM
• On-call duties: 4-6 nights per month, approximately 1 weekend per month
