Outpatient Primary Care Practice Description Established primary care practice serving a diverse patient population

Mix of adult and pediatric patients

Strong referral network with access to specialists and hospital services

Collaborative and supportive clinical environment Clinical Responsibilities Provide comprehensive primary care services to patients of all ages Diagnose and treat acute and chronic medical conditions

Perform preventive care, health maintenance, and wellness visits

Manage chronic disease states such as diabetes, hypertension, and asthma

Coordinate care with specialists and ancillary services as needed

Maintain accurate and timely medical documentation in the electronic medical record

Participate in quality improvement initiatives and evidence-based care Schedule Full-time outpatient schedule

Monday-Friday clinic hours

Shared or minimal call responsibilities, depending on practice needs

No or limited inpatient responsibilities Requirements

MD or DO degree from an accredited medical school

Completion of an ACGME- or AOA-accredited Family Medicine residency

Board Eligible or Board Certified in Family Medicine Active Pennsylvania medical license or ability to obtain

Strong communication and patient-centered care skills Support & Resources Experienced clinical and administrative support staff
