Provides compassionate, hands‑on nursing care focused on comfort, dignity, and quality of life for individuals with life‑limiting illnesses. Working under the supervision of a Registered Nurse as part of an interdisciplinary team, the LPN delivers skilled care such as medication administration, symptom monitoring, wound and catheter care, and patient education, primarily in patients’ homes or partner facilities across central and south‑central Kansas.
The role also involves supporting patients and families emotionally, communicating changes in condition to the care team, and ensuring care aligns with each individual’s goals and Midland Care Connection’s mission of innovative, person‑centered, community‑based hospice care.
- Current Licensed Practical Nurse (LPN) license in Kansas
- Graduation from an accredited practical nursing program
- Basic Life Support (BLS/CPR) certification
- Ability to provide compassionate end‑of‑life care
- Strong communication and documentation skills
- Valid driver’s license with reliable transportation and auto insurance
- Previous hospice, palliative care, home health, or long‑term care experience
- Experience working with older adults or seriously ill patients
- Knowledge of symptom management and comfort‑focused care
- Ability to work independently in home and facility settings
- Familiarity with interdisciplinary team‑based care
- Strong empathy, emotional resilience, and commitment to patient‑centered care
