Job Summary

Under the supervision of a Registered Nurse or Clinician, the Licensed Practical Nurse (LPN) in the respite care setting delivers high‑quality, culturally appropriate, and patient‑centered care. This role involves providing transitional care for patients recovering from acute illnesses or injuries, ensuring continuity of care and addressing medical, psychosocial, and recovery needs, promoting health stability and continuity of care. The LPN contributes to planning, implementing, and evaluating care plans while adhering to organizational and regulatory standards.

Key Responsibilities
  • Patient Care & Flow:
    Provide direct, age‑specific care, administer medications and treatments, monitor and document vital signs, coordinate room turnover, perform venipuncture and simple labs.
  • Care Coordination:
    Collaborate with interdisciplinary teams to develop care plans, track follow‑ups in EMR, assist with scheduling and accessing ancillary services, facilitate discharge planning.
  • Health

    Education:

    Teach patients and families self‑care, medication adherence, and chronic disease management.
  • Emergency Response:
    Recognize medical emergencies, perform CPR and triage, contact supervising RN/clinician as needed.
  • Equipment & Supplies:
    Clean and prepare medical equipment, maintain inventory, operate pulse oximeters, nebulizers, glucometers, and CLIA‑waived lab equipment.
  • Quality Improvement:
    Participate in QA/QI activities and collaborate on organizational initiatives to optimize care delivery.
  • HIPAA Compliance:
    Access and use PHI per minimum‑necessary guidelines, schedule appointments, coordinate care.
  • Administrative Support:
    Assist with patient scheduling, filing, and other administrative duties, triage telephone inquiries under supervision.

    Engage in staff training, case conferences, and contribute to an inclusive workplace culture.

Education and Experience
  • Current Oregon Licensed Practical Nurse License.
  • Current CPR (health care provider level).
  • Preferred: 1 year experience as an LPN.
  • Preferred:
    Proficiency in English and Spanish, written and spoken.
  • Preferred:
    Previous work in respite care, transitional care, or community health.

Immunization
