Job Summary and Responsibilities

We are seeking an RN Complex Case Manager (Hybrid) in Lebanon, OR or Abingdon, Virginia. The RN Complex Case Manager will provide in‑home, in‑person care coordination and telephonic care coordination across the United States, focusing on coal mine states. This role utilizes an integrated case management model that incorporates case and disease management to maintain beneficiaries at their optimal functional level.

Key responsibilities include:

  • Provide in‑home and telephonic care coordination, ensuring an integrated case management model that supports beneficiary stability and functional improvement.
  • Maintain the Funds’ “caring” philosophy, coordinating care with beneficiaries and providers to maximize functional capacity and improve quality of life.
  • Assess, plan, implement, and evaluate individualized plans for beneficiaries across the continuum of care.
  • Facilitate integrated case management and disease management activities in line with CMSA definitions and principles.

NOTE: This is a full‑time, direct hire, exempt, hybrid opportunity with benefits. The selected candidate must reside within a one‑hour drive of Lebanon, OR or Abingdon, VA. Phone and travel for in‑home visits are required: up to 25% in‑home visits and 75% telephonic care. Hours are 8:00 AM – 5:00 PM Eastern, Monday–Friday.

Responsibilities:

  • Use independent nursing judgment to address and resolve clinical issues during home visits, including medication set‑up and vital sign checks.
  • Consult with physicians, specialists, pharmacists, and other disciplines to optimize beneficiary function and prevent health decline.
  • Develop beneficiary‑centered care plans with shared accountability among beneficiaries, caregivers, and providers.
  • Coordinate health and social services, coach beneficiaries and families, advocate, educate, and communicate status and plans among the care team.
  • Conduct home visits and visits to hospitals, nursing homes, and physicians’ offices as needed to continue care plans and support transition.
  • Review care plans in regular care conferences, emphasize transitions, and teach self‑management for chronic conditions.
  • Ensure day‑to‑day processes comply with URAC and other regulatory standards.

Qualifications

Required:

  • Unrestricted, active RN compact state license.
  • High School diploma or GED equivalent.
  • 3+ years of clinical experience with at least 1 year of case management experience.
  • Proficiency with Microsoft Office, internet/web navigation, and research.
  • Experience with electronic medical record systems and multiple computer applications.
  • Ability to perform basic physiological assessments without physician orders.

Preferred:

  • Bachelor of Science in Nursing (BSN) or higher.
  • Public health or geriatric nursing experience.
  • Certified Case Manager (CCM).
  • Strong multitasking, prioritization, and decision‑making skills.
  • Strong communication, interpersonal, organizational, and time‑management skills.
  • Ability to work independently and as part of a team.
  • Interest in continuous learning and staying informed on regulatory changes.

Compensation

Pay range: USD $67,300.00 – USD $74,000.00 per year.
