Seeking LVNs in Collin CountyWe are looking for Licensed Vocational Nurses (LVNs) to provide direct nursing care to patients in the Collin County area.
Position Responsibilities- Provides direct nursing care according to the medical plan of treatment and nursing care plan.
- Utilizes physical and psychosocial assessment skills, performs multi-system assessments and identifies current and potential care needs.
- Carries out physician orders for treatment and visit frequency per plan of treatment.
- Organizes and completes patient treatment and tasks per established care plan.
- Implements and updates care plan in collaboration with the Clinical Manager and Primary Care Clinician to reflect progress toward goals and outcomes.
- Coordinates care among team members and documents in the clinical record per policy.
- Educates and trains patients and caregivers on exercises and equipment needed to aid recovery.
- Provides education and instruction specific to patient and caregiver assessed needs, abilities and readiness.
- Accurately documents ongoing assessments, clinical findings, skilled care and plan for next visit according to the established care plan.
Must be flexible with schedule including, but not limited to, the ability to participate in on call and/or department rotation for night and weekend coverage.
Qualifications & Requirements- Strong organization skills and time management.
- One year of previous geriatric home health experience required.
- Able to multitask and accomplish and meet timelines.
- Professional appearance and effective communication skills.
- Current Texas Registered Nurse (LVN) license.
- One year of clinical nursing experience.
- Current Texas driver's license.
- CPR Required.
- Experience with healthcare software (Kinnser preferred).
- One year of previous OASIS experience.
- Competency to carry out any skilled procedure that the physician orders that is within the scope of the Nurse Practice Act.
- PICC Line and Wound Vac Experience (preferred).
