Medical City Dallas invites applications for the Pediatric Nurse Navigator
- Congenital Heart role, an on‑site position focused on pediatric congenital heart patients. The schedule is Monday‑Friday 8:00 AM‑5:00 PM.
The Nurse Navigator functions as a member of the multidisciplinary team and serves as an advocate and educator for patients with all types of medical conditions. The primary function is to support patients through complex healthcare environments to optimize care and outcomes.
Key responsibilities include:
- Coaching, information transfer, and communication to improve continuity of care.
- Assisting with appointments, resolving barriers, and providing education.
- Connecting patients and families with community resources.
- Managing resources to achieve efficient, high‑quality outcomes.
- Collecting data to track navigation program performance in line with HCA Healthcare goals.
- Identifying opportunities to streamline care practice through evidence‑based guidelines.
Education:
- Bachelor’s Degree preferred.
Experience and
Certifications:
- 3+ years of clinical experience – Required.
- 3+ years as a Nurse Navigator and/or patient educator in the service line – Preferred.
- 3+ years minimum Peds/PICU experience – Preferred.
- Basic Cardiac Life Support – Required.
- Registered Nurse license in the state of practice – Required.
- Service Line Specialty Certification – Preferred.
