Job Summary

Under the supervision of the Diversion Services Supervisor, the Jail Peer Support Specialist works closely with jail partners to support adult individuals identified in jail with a severe mental illness (SMI) or co-occurring SMI and substance use disorder who will be transitioning from jail to the community. The goal is to improve quality of life, support physical and mental health stabilization, and reduce recidivism.

Responsibilities
  • Engage clients and provide peer support services using a trauma‑informed, person‑centered approach.
  • Perform Continuity of Care Query (CCQ) screenings and conduct skills training and psychosocial rehabilitation services.
  • Coordinate care to ensure timely access to quality services, working with the Life Path Systems team and community organizations to reduce barriers.
  • Assist clients with benefit applications and housing resources and support medication continuity during the transition from incarceration to community.
  • Document services promptly, maintain organized scheduling, and participate in treatment team meetings.
  • Build and maintain positive relationships with jail staff, mental health professionals, and other community stakeholders while demonstrating professional conduct at all times.
  • Assess and de‑escalate behavioral health crisis situations calmly and respectfully.
  • Meet performance targets: 90% of services notes signed within 2 business days, 50% of work time in direct service hours, 90% of chart reviews score 80% or higher, and all required trainings completed before due dates.
Qualifications
  • High School Diploma or GED.
  • At least one year of experience in mental health community services with a primary diagnosis of mental illness.
  • Minimum one year of active recovery and/or sobriety.
  • Eligible to become a Certified Peer Specialist within 12 months of employment.
  • One year of prior experience in a mental health field and jail setting is preferred.
  • Reliable transportation and valid Texas Driver’s License.
  • Bilingual Spanish/English preferred.
  • Knowledge of community resources and program policies and procedures.
  • Strong ability to assess needs, coordinate services, communicate effectively, and train others.
  • Experience providing case management, skills training, psychosocial rehabilitation, and symptom management to individuals with mental illness.
  • Ability to complete detailed, concise progress notes and meet timely documentation standards.
  • Good time management and organizational skills.
  • Capable of working independently and collaboratively, with effective communication with supervisory staff.
