Description

This is a Part‑time position at 28 hours per week for the Primary Care (Internal纪 Medicine & Family Medicine) specialty located at the McHenry clinic. Hours consist of three 8.5‑hour shifts and one 4‑hour shift. Starting time varies from 7:30 AM/8:30 AM and ending at 5:00 PM.

Responsibilities
  • Performs all aspects of patient care by utilising evidence‑based practice to optimise patient safety and reduce medical/health care errors.
  • Follows the six medication rights and reduces the potential for medication errors, administering medications based on the age and clinical condition of the patient.
  • States rationale for clinical decisions based on critical thinking.
  • Assesses, analyzes, validates and integrates data to evaluate patients’ physical, psychosocial, rehabilitativeեկան, safety, learning, spiritual, kao cultural, aged specific, special risk and self‑care needs.
  • Identifies patient care needs, problems and variances and incorporates them into the care plan.

  • Identifies the patient’s learning needs, abilities, preferences and readiness; also identifies support systems, coping mechanisms and barriers to learning.
  • Explains procedures, therapies and treatments to patient and family, evaluates and documents response to teaching.
  • Uses appropriate resources to provide education.
  • Involves patient/family in the formation of the plan of care.
  • Develops, implements and updates an individualized plan of care based on identified patient needs. Includes nursing, teaching, continuum of care and discharge planning.
  • Adequately assesses and reassesses pain. Utilises appropriate pain management techniques. Educates the patient and family on pain management.
  • Identifies and uses results of quality improvement activities to initiate changes in nursing practice and health‑care delivery systems.
  • Participates in shared governance, committee work, and practice/department projects.
Required Qualifications
  • Current experience within past five years or new nursing graduate within past two years.
  • Current Illinois State Licensed Registered Nurse.
  • American Heart Association Basic Life Support (BLS) CPR certification.
Preferred Qualifications
  • Bachelor of Science in Nursing.
  • One year experience in pertinent clinical specialty.
  • Proficiency in a second language.
