Position Summary

The Licensed Practical Nurse (LPN) or Registered Nurse (RN) reports directly to the Director of Nursing and delivers, coordinates, and evaluates resident care in alignment with physician orders, individual care plans, and regulatory standards. The role combines hands‑on clinical responsibilities—medication administration, assessments, documentation, and direct resident support—with leadership functions such as guiding care staff, promoting safety initiatives, and ensuring effective communication with residents, families, physicians, and community leaders.

  • Provide services in accordance with nursing practice act and state/federal regulations.
  • Collaborate with Director of Nursing to give direction and oversight to the care staff regarding resident needs.
  • Review and revise assessments and care plans at admission, periodically per regulation, and when resident condition changes.
  • Transcribe and apply physician’s orders, assisting physicians with resident examinations as needed.
  • Develop resident education plans per physician and community policy; provide instruction to residents and families.
  • Observe residents and communicate condition changes to physicians, health‑care proxies, and supervisors, providing timely interventions.
  • Administer and document medications following the five rights: right medication, right route, right resident, right time, and right dose.
  • Supervise and monitor medications for residents who self‑administer, if applicable.
  • Document all observations, incidents, interventions, communications, clinical procedures, and medication assistance throughout the shift.
  • Assist residents with nutritional and hydration needs and record data on the nursing record.
  • Support activities of daily living—bathing, showering, oral care, toileting, dressing—as needed.
  • Assist residents with scheduling medical, dental, and laboratory appointments.
  • Use proactive interventions, monitoring, and safety measures in rooms and during rounds.
  • Answer residents’ call light promptly.
  • Assist with resident transfers and ambulation using proper body mechanics.
  • Take vital signs to detect deviations from normal and assess resident condition as requested.
  • Maintain a restraint‑free environment at all times.
  • Support fall prevention programs for all residents in need.
  • Respond to life‑saving situations according to nursing standards, policies, procedures, and protocols.
  • Respond appropriately to personal calls from other health‑center units.
  • Provide first aid, call emergency personnel (dial 911), and notify family members per resident request.
  • Review resident service plans for assisted living and memory care.
  • Encourage residents to participate in scheduled facility activities, helping them attend as needed.
  • Assist in post‑mortem care as instructed.
  • Oversight and management of medication delivery system, including order entry, receipt, tracking, MAR documentation, narcotic counts, and timely medication checks.
  • Notify pharmacy and supervisor of issues and seek resolution promptly.
  • Promote physical, personal, and emotional well‑being of each resident, striving to return them to maximum self‑care when possible.
