• Provide direct nursing care and assist in developing individualized treatment plans.

• Oversee resident safety programs, including Falls and Pressure Sore prevention.

• Conduct and document medical assessments and progress updates.

• Collaborate with interdisciplinary teams to ensure excellent resident outcomes.

• Active LPN license in good standing.

• Experience in skilled nursing or rehabilitation preferred.
