Join our amazing team of healthcare providers. We have been in business for over 10 years. As we expand we are looking for highly motivated LVN's to join our Private Duty Nursing team.

• Provide skilled nursing care under the direct supervision of RN.
• Under the direction of the RN, assists in identifying the patient's / client's physical, psychosocial, and environmental needs as evidenced by documentation.
• Participates in planning and implementing care in conjunction with the RN, in accordance with the POC.
• Provides patient / client and caregiver education.
• Provides care using infection control measures that protect both staff and patient / client.
• Assures the continuity of care through the delivery of quality patient / client care.
• Provides effective communication to patient / client / family, team members, and other health care professionals as evidenced by clinical notes, case conferences, and communication notes. Completes appropriate documentation in a timely manner
• Monitors assigned cases to ensure compliance with physician orders and requirements of third party payor.
• Demonstrates commitment, professional growth, and competency.
• Perform other duties as assigned.
