Field Pediatric RN Case Manager
Health Care Support is actively seeking a Field Pediatric RN Case Manager to fill an opening with a Fortune 100 Managed Care company in the McAllen/Edinburg area
.
- Completing assessments (Biannuals, Annual Reassessments)
- Telephonic outreaches
- Care coordination
- Follow up on request from members/LAR
- Complete any assigned trainings
- Completing all required documentation
- Submitting authorizations
- Travel (home visit to complete assessments)
- Maintaining audit scores above 92%
- Providing member/LAR education
- Completing Productivity reports
- Follow up on HEDIS measures
- Assisting members/LARs with finding PCPs/Specialists, Therapy agency, PCS agency, DME and etc
- This will be a Contract position for at least 6 months with potential to convert into a permanent role and target start date will be ASAP
- Schedule for the position is Mon-Fri 8-5, must be flexible with hours (may work 5-10 hours of overtime needed to go see a member after hours when they're out of school)
- 4 years of experience as a Registered Nurse
- 2+ years of Pediatric experience required preferably working with children with disabilities and vulnerable population (this team works with children with special needs and with the IDD/Intellectual and Developmental Disability Waivers, conditions include , Autism, Cerebral Palsy, Down Syndrome, Fetal Alcohol Syndrome, developmental delays, spina bifida, deaf or blind)
- Home health experience required, case management preferred
- Ability to learn new technologies and enter assessments into program as they’re being conducted
- Clear and active TX RN license
- Clear and active driving record, reliable transportation, and insurance
- Hands off assessments; rely on nursing skills to complete holistic assessment
- Bilingual in Spanish is Highly Preferred
