We are looking for a dependable, patient‑centered clinician committed to providing evidence‑based, individualized care.

• Conduct Patient Evaluations:
  Perform comprehensive assessments to determine functional abilities, home safety needs, and individualized goals.
• Develop Individualized Care Plans:
  Create patient‑specific treatment plans focused on improving ADLs, fine motor skills, cognitive function, and overall independence.
• Patient & Caregiver
  
  Education:
  
  Educate patients and caregivers on home exercise programs, adaptive equipment use, safety strategies, and techniques to support continued progress.
• Monitor and Document Progress:
  Complete accurate and timely documentation of visits, patient responses, changes in condition, and updates to care plans.
• Deliver Compassionate, High‑Quality Care:
  Provide empathetic, supportive, and goal‑focused treatment to enhance quality of life and functional independence.

• Active Occupational Therapist (OT) license in the state of Texas (TX)
• Home health or rehabilitation experience preferred
• Strong clinical reasoning and ability to tailor treatment to individual needs
• Knowledge of evidence‑based therapy practices and standards of care
• Strong communication skills and a professional, patient‑centered approach
• Valid Driver’s License
• Kinnser/Well Sky EMR experience preferred
