Join our compassionate team and make a difference in the lives of our patients! We are seeking a skilled LVN to deliver high-quality nursing care in patients’ homes, under the supervision of a Registered Nurse. This role involves collaborating with the healthcare team, patients, and families to ensure the best outcomes.

Job Title: LVN – Adult Hospice and Home Health

Locations: Lower Valley & Upper Valley

Job Types: Part-Time, PRN

Shifts: Day Shift, Evening Shift, Night Shift, and Weekends as needed

Work Schedule: Monday to Friday, Weekends, Evenings as needed

Key Responsibilities
  • Patient Care:
    Provide direct nursing care to patients in their homes, ensuring the implementation of care plans in line with physicians’ orders.
  • Assessment & Documentation:
    Regularly monitor vital signs, track medical conditions, document changes, and maintain accurate health records.
  • Medication Management:
    Monitor patients’ medication usage and report results, ensuring proper administration and dosage.
  • Collaboration:

    Work closely with the interdisciplinary team, patients, and their families to assess, implement, and evaluate care plans.
  • Personal Care:
    Assist with emotional, personal, and medical support for patients, providing holistic care to meet their needs.
  • Care Coordination:
    Evaluate patient behavior, diagnose conditions, and consult with healthcare professionals to improve patient outcomes.
Working Conditions
  • Work in patient homes, often in varying environmental conditions (including possible exposure to bodily fluids and infectious diseases).
  • Ability to travel locally to patient homes and manage flexible work hours.
  • Occasional emergency PRN calls and exposure to adverse weather.
  • Reasonable accommodations may be made for individuals with disabilities.
Qualifications
  • Education:

    Graduate of an accredited school of vocational nursing.
  • License/

    Certifications:
    • Current state LVN License.
    • BLS Certification (required).
    • Valid Driver’s License.
  • Experience:

    Minimum of 2 years as an LVN in Clinical, Hospice, or Home Health settings (preferred).
