Position: Registered Nurse for Home Health

Advance Home Health is a joint-commission accredited home health care service committed to providing accessible and high-quality care to clients in the greater Chicagoland area. Our team includes skilled nurses, physical therapists, occupational therapists, and aides who deliver a wide range of healthcare services.

Role Description

The agency's Registered Nurse provides skilled nursing care services by organizing, planning, directing, coordinating and evaluating all aspects of nursing care in accordance with a physician's plan of care through intermittent visitation in the client's home. RNs are responsible for teaching and supervising client family members on continuity of care and comprehensive care.

We are currently looking for RNs to visit patients in the following counties:

  • De Kalb
  • Kane
  • Kankakee
  • Lake
  • Mc Henry
  • Will
Qualifications
  • Graduate of an accredited school of nursing *
  • Experience in home health care or public health nursing preferred, but not required
  • Possession of excellent written and verbal communication skills
  • Excellent organizational and leadership skills

* Must be a licensed Registered Nurse in the State of Illinois

Accountability

When assigned to a home care client, the RN is accountable to the agency, Director of Health Care Services/Home Care Supervisor.

Responsibilities

The registered nurse performs acts that require substantial specialized knowledge, judgment, and nursing skills based on principles of psychological, physical, and social services. RNs are responsible and accountable for making decisions that are based on their educational preparation and experience in nursing.

Ensures the quality and safe delivery of home health care services

  • Completes nursing assessment forms in a timely manner per agency policy and procedures
  • Identifies physical, emotional, and developmental client needs
  • Develops individual nursing care plan that reflects current client needs prior to case conference
  • Provides health teachings to clients and their families
  • Makes proper and accurate clinical record documentation that reflects delivery of quality and safe home health care services

Implements current practice

  • Assesses client condition on every home visit;
  • Communicates client assessments to appropriate individual concerns
  • Implements nursing interventions to meet client needs and changed conditions
  • Documents accurate clinical record on every home visit per agency policy and procedure
  • Accurately transcribes, evaluates, and implements physicians plan of care in a timely manner
  • Refers clients to other health care providers per agency policy
  • Implement infection control and safety measures per agency procedure and policy
  • Implement clinical care per agency policy
  • Monitors client for compliance with defined care plans

RNs may be asked to fulfill additional responsibilities as assigned when necessary and qualified to perform. Performances include, but are not limited to, the following;

  • Initiation of/administration of IV therapy
  • Insertion and irrigation of nasogastric tubes
  • Suction and cleaning of tracheal and endotracheal tubes
  • Drawing blood for laboratory testing
  • Cardiopulmonary resuscitation

Assumes assigned responsibility for home healthcare agency operations in the absence of the Agency Supervisor and Field Supervisor

  • Complies with federal, state, and local regulations for home healthcare agency operation
  • RNs must be available at all times during agency operating hour (RN supervisor is available at all times during these hours)

Supervises home health care LPN (Licensed Practical Nurse) and Home Health Aides

  • Supervises Home Health Aides and LPN every two (2) weeks
  • Monitors job performance of aides and LPN - reports about job performance of LPN and Aides to Director of Nursing/Agency supervisor
  • On-call for emergencies involving Home Healthcare staff members and clients
  • Participate in a monthly home healthcare orientation and staff meetings
  • Participates in appropriate committee meetings
  • Cooperates and communicates with Home health staff to foster a positive and productive work environment (open communication about availability, only accepting assignments for which you are qualified)
  • Consults with Agency supervisor or Director of Nursing when issues or concerns arise (ex: documentation for equipment and supply charges)
Physical Requirements
  • Visual/hearing ability sufficient to comprehend written and verbal communication
  • Ability to perform physically strenuous tasks (may include heavy lifting, extended periods of standing)
