Your Responsibilities

• Conduct comprehensive medical and functional assessments of patients in skilled nursing and rehabilitation facilities.
• Collaborate with an interdisciplinary team?including physicians, physical therapists, and occupational therapists?to develop and monitor individualized care plans.
• Manage and evaluate patient progress, addressing medical barriers to recovery and optimizing outcomes.
• Participate in daily clinical rounds and coordinate with nursing staff and facility administrators to ensure quality care delivery.
• Document visits and care plans using the Doctor Now EHR, integrated with Point Click Care .
• Provide ongoing communication with the primary care team, contributing to chronic care management and readmission prevention programs.
• Maintain communication with regional clinical supervisors to ensure clinical excellence and team collaboration.

Qualifications

• Licensed Nurse Practitioner (NP) or Physician Assistant (PA) in Massachusetts
  .
• Strong interpersonal, communication, and clinical assessment skills.
• Experience or interest in Physical Medicine & Rehabilitation (PM&R) preferred.
• Ability to work independently while collaborating with a multidisciplinary team.
• Passionate about patient-centered care and improving outcomes.
