Position: Nurse Supervisor, McCarthy Care Center, (Per Diem)
Location: East Sandwich

Purpose of Position

Purpose of Position The Hospice Nurse Supervisor, under the direction of the Clinical Manager, is responsible for (i) planning, coordinating, implementing, and evaluating patient care services and staff education related to hospice care to provide for the safety and quality of care, treatment or services for patients in the McCarthy Care Center and (ii) the supervision of all clinical staff including, but not limited to, LPNs and Hospice Aides.

Responsibilities
  • Oversee hospice program development in collaboration with senior leadership, including interview and hiring of staff, staff education, data management, and implementation of appropriate clinical practices.
  • Determine staffing needs according to census requirements and with consideration for utilization of personnel at their maximum potential.
  • Oversee compliance with personnel policies and monitor day‑to‑day compliance with regulatory and reimbursement requirements as defined by various government agencies.
  • Maintain compliance with all Joint Commission, state, and federal regulations and assist with the orientation, preceptorship, and ongoing education of all McCarthy Care Center staff.
  • Collaborate with other departments to develop, maintain, and monitor policies and standards related to hospice activities and determine patient eligibility for General Inpatient level of care within the program guidelines established in collaboration with hospice medical director, interdisciplinary team members, and hospice intake department.
  • Perform the admission process, including all signed forms, physician orders, and CTI’s completed, and establish patient plan of care including medication reconciliations.
  • Initiate the GIP (General Inpatient) Plan of Care based on the patient and family/caregiver needs, reflect assessment data obtained to determine the nursing diagnosis, and establish goals for care. Include both the patient and family/caregiver in the care planning process, re‑evaluate this Plan of Care regularly, and revise as needed to reflect the patient and family/caregiver care needs.
  • Manage patient symptom management; administer medications and treatments as prescribed by the physician.
  • Attend the Interdisciplinary Group Team meetings regularly and participate in hospice‑sponsored in‑service training and mandatory trainings.
  • Ensure that equipment, medication and other necessary items and services are available to the patients in the McCarthy Hospice Care Center.
  • Provide clinical expertise and serve as a resource to staff and the community, participate in Agency Quality Improvement program, and consistently provide service excellence to all patients, family members, visitors, volunteers, and co‑workers in a manner that reflects VNA’s commitment to CARES: compassion, accountability, respect, excellence, and service.
  • Other duties as assigned.
Qualifications
  • Registered Nurse currently licensed in the Commonwealth of Massachusetts.
  • BSN, Master’s degree in Nursing preferred.
  • Minimum of two to four years of supervisory experience in a home health agency, hospice or related health program.
  • Ability in program administration.
  • Excellent oral and written communication skills and ability to utilize available hardware and software.

Per Diem Hours McCarthy Care Center, Mon‑Fri shifts 7a‑3p, 3p‑11p, 11p‑7a; wknds 12 hr 7‑7. Occ. Weekends & Holidays
