Certification Details
- Providing clinical services to patients from birth to over 100 years in their homes.
- Assessing patient conditions, implementing care plans, and providing ongoing education and support to patients and their families.
- The admissions process and case management responsibilities are performed in accordance with MGB Healthcare at Home’s policies and standards of practice, utilizing thorough and timely electronic documentation and interdisciplinary communications.
Job Details
- 1 year Certified Home Care experience
- 1 year Case Management Experience
- Current licensure in the state of Massachusetts
- Reliable, independent means of transportation to make home visits.
- Must be willing to work weekends/holidays/on call in accordance with department need/rotation.
- Weekend rotations are once every 6 weeks.

Schedule Information
- MGB Badge Photo required for onboarding.
- MGB MVR Check required within 45 days of start.
- License/Certificate Verification required for submission; MA Conditional Licenses not accepted.
- N95 Fit Test within 365 days to start date and updated annually.
- MGB Nurse Aide Registry check within 30 days of start.
- Documentation of MMR vaccination or immunity required.
- Hepatitis B vaccination strongly preferred.
- Influenza Vaccine required during flu season (Sept 15 - April 30).
- TB Test documentation within 3 months of start date and updated if >6 month break.
- Health Statement signed by MD/DO/NP/PA within 365 days of start date.
- MGB Criminal Background Check within 45 days prior to start date.
- MGB Massachusetts CORI check within 30 days prior to start date.
- MGB Resume including past 7 years of work experience required for submission.
- Respirator Medical Clearance Attestation required if fit test is required.
- MGB References required for submission.
- MGB Skills Checklist required for submission.
- MGB Confirmation of Assignment/POI Form required for all initial assignments and extensions.
- Positive PPD requires negative chest x-ray documentation.
- MGB Education Verification required for onboarding.
- MGB SSN Trace required for onboarding.
- MGB National Sex Offender Check required within 30 days of start and updated annually.
- MGB Exclusion Checks required within 30 days of start and annually.
- MGB TB Symptom Survey if Positive PPD is optional but recommended.
- MGB Candidate Submission Coversheet required for submission.
