OCHS now cares for more than 18,000 patients annually and no one is denied access to services due to an inability to pay. Founded in 1987 through the merger of Health Associates of Provincetown (established in 1972) and the AIM Medical Center in Wellfleet (established in 1966), Outer Cape Health has a long history of successful growth and expansion in pursuit of this mission.

The Primary Care Access Clinician at Outer Cape Health Services (OCHS) ensures patients of all ages have access to comprehensive same‑day care, chronic disease management, and temporary coverage during primary care clinicians’ leaves of absence (planned and unplanned). They provide routine and urgent medical care as needed. While not assigned to a specific patient panel, they assist with panel management needs of primary care patients as directed by the medical director.

• Serve as part of a Patient Centered Medical Home team to ensure consistent coverage and access for OCHS patients to primary care and same‑day care medical services, including regular communication with primary care clinicians and clinical teams regarding patient health.
• Supervise other health care professionals such as staff registered nurses, medical assistants, phlebotomists, and allied health staff, as appropriate, to ensure proper delivery of medical care.
• Collaborate with the practice manager/director of operations and the medical director regarding daily/weekly schedule which will change depending on coverage needs.
• Medically manage or support the care of emergency, urgent, and other walk‑in patients and cooperate with site staff in managing such needs.
• Keep accurate, complete, legible and timely medical and other records and files as required by OCHS policy.
• Participate in peer review and patient care audits for quality assurance and utilization review to ensure an excellent standard of care.
• Participate in on‑call duties as assigned.
• Maintain knowledge of current developments in specialty to enhance professional expertise, including maintenance of Board Certification and completion of continuing medical education credits as needed.
• Maintain the highest standard of professionalism toward colleagues, patients and all OCHS stakeholders.
• Participate in clinician and staff meetings as set by OCHS leadership and management teams.
• Perform other related duties as reasonably required or assigned.
• Adhere to OCHS policies and procedures.

• Family Medicine Clinician with Advanced Practice / Master’s Degree in Nursing or Physician Assistant training.
• Valid Nurse Practitioner or Physician Assistant license to practice medicine in the Commonwealth of Massachusetts.
• Basic Life Support (BLS) certification.
• Experience working in fast‑paced ambulatory care settings; FQHC/Community Health training/experience preferred.
• Possess a valid federal Drug Enforcement Administration number and other required state public health certifications with respect to prescribing privileges.
• Certified clinician and accepts assignment under the Medicare and Medicaid programs; qualified clinician with respect to third‑party payers.
• Is not excluded by state or federal authorities from participating in any health care programs, including Medicare and Medicaid.
