What Youll Do
  • Resident-Centered Assessment:
    Conduct initial and ongoing assessment of residents’ physical and emotional health. Document changes and communicate them to the healthcare and dietary teams.
  • Care-Planning:
    Collaborate with the healthcare team to develop and update individualized care plans based on residents’ needs and preferences. Participate in care plan meetings to discuss progress and adjustments.
  • Supervision and Delegation:
    Oversee and delegate tasks to CNAs and Resident Assistants, providing guidance to ensure safe and effective performance.
  • Assistance with ADLs and Procedures:
    Provide assistance with activities of daily living and coordinate/perform medical procedures and treatments (e.g., wound care, injections, monitoring chronic conditions) under supervision of the Wellness Director.
  • Documentation:
    Maintain detailed, up-to-date medical records, including vital signs, medication administration, and changes in health status to support oversight of resident care.
  • Staff Collaboration and Environment:
    Contribute to a warm, positive atmosphere by building relationships with residents and participating in team activities as appropriate.
What You Bring
  • Education:

    Completion of a state-approved practical nursing program; licensure as an LPN in the state; program accredited by the state nursing board or a recognized national accrediting agency.
  • Professionalism:
    Maintain confidentiality, respect cultural differences, and adhere to ethical standards in interactions with residents, families, and teammates.
  • Clinical Expertise:
    Proficiency in medication administration, wound care, vital signs monitoring, and other essential clinical skills to provide competent care.
  • Team Player:
    Value collaboration with registered nurses, CNAs, and other staff to ensure a seamless care experience.
  • Lifelong Learning:
    Commitment to ongoing learning and professional development in senior care.
Working Conditions & Physical Demands

As an LPN at Dominion Senior Living, you will provide compassionate care in a comfortable, collaborative work environment. The role may require standing, bending, lifting up to 50 pounds, and assisting residents with transfers, along with vision and sensory requirements as needed.
