- Resident‑centered assessment – Conduct initial and ongoing assessment of residents’ physical and emotional health, document changes, and communicate with the healthcare and dietary teams.
- Care‑planning – Collaborate with the healthcare team to develop and update individualized care plans, and participate in care‑plan meetings.
- Supervision and delegation – Oversee and delegate tasks to CNAs and Resident Assistants, providing guidance and direction.
- Assistance with ADLs and procedures – Provide assistance, coordinate medical procedures such as wound care, injections, and monitor chronic conditions under the Wellness Director’s guidance.
- Documentation – Maintain detailed medical records, including vital signs, medication administration, and health status changes.
- Build positive relationships – Create a warm and welcoming atmosphere through activities and meaningful interactions.
- Education – Completion of a state‑approved practical nursing program; LPN graduation from an accredited program.
- Professionalism – Maintain confidentiality, respect cultural differences, adhere to ethical standards.
- Clinical expertise – Proficient in medication administration, wound care, and vital‑sign monitoring.
- Team player – Collaborate with RNs, CNAs, and other staff for seamless care.
- Lifelong learning – Pursue ongoing education to provide the best care possible.

Physical DemandsThe job requires standing, bending, walking, sitting, climbing stairs, stooping, using hands and fingers to touch, handle, and feel, reaching with arms, lifting or moving items up to 50 pounds, and occasionally lifting and transferring residents. Vision requirements include close, distance, color, peripheral, depth perception, and the ability to smell odors for safety.
