Overview
Health and Wellness Director (Project Management) — Full-Time
• Exempt
Role:
Lead the clinical and wellness operations of our Assisted Living and Memory Care community. Ensure high-quality resident care, regulatory compliance, team development, and a resident- and family-centered environment in accordance with Tennessee Assisted Living Facility regulations ).
Reporting to the Executive Director (with a dotted line to the Divisional Director of Operations).
Responsibilities- Oversee daily Assisted Living and Memory Care clinical operations
- Ensure compliance with Tennessee ALF regulations
- Lead quality assurance, audits, and performance improvement initiatives
- Prepare for and respond to Tennessee Department of Health surveys
- Maintain and implement policies and procedures
- Coordinate resident care, assessments, and individualized service plans
- Oversee medication administration and RN/LPN delegation
- Coordinate care with physicians, pharmacies, and therapy providers
- Ensure accurate, timely clinical documentation
- Memory Care oversight: ensure compliance with dementia training and staffing requirements; oversee therapeutic programming and safety protocols; ensure individualized service plans meet cognitive and behavioral needs
- Staff leadership & development: hire, train, supervise, and evaluate staff; ensure all required competencies and training; promote teamwork, professionalism, and effective communication
- Resident & family relations: serve as primary contact for clinical and wellness concerns; conduct care conferences; address resident/family concerns; support smooth move-ins and care transitions
- Risk management & safety: oversee infection prevention, fall reduction, and safety programs; investigate incidents and implement corrective actions; ensure emergency preparedness compliance
- RN preferred; LPN considered with strong leadership experience
- Minimum of 2 years" experience in Assisted Living, Memory Care, or Long-Term Care
- Strong knowledge of Tennessee ALF regulations
- Excellent leadership, communication, and organizational skills
- Active RN or LPN license in the state of Tennessee
