Job Summary: The Physical Therapist at Marysville Post Acute will be responsible for evaluating, planning, and administering physical therapy treatments to help patients recover, improve mobility, and regain strength and function. The ideal candidate will work closely with a multidisciplinary team to develop individualized treatment plans and ensure the highest standard of care for our residents.

Key Responsibilities:

  • Conduct comprehensive physical therapy evaluations to assess patients' physical condition and functional abilities.
  • Develop individualized treatment plans based on assessment findings, patient goals, and best practices.
  • Administer physical therapy treatments, including exercises, manual therapy, modalities, and other interventions to improve strength, flexibility, balance, and coordination.
  • Monitor and document patient progress, adjusting treatment plans as necessary to achieve optimal outcomes.
  • Educate patients and their families on treatment goals, therapeutic exercises, and strategies for continued progress after discharge.
  • Collaborate with other healthcare professionals, including physicians, nurses, occupational therapists, and speech therapists, to ensure a cohesive approach to patient care.
  • Maintain accurate and timely documentation of patient care, in compliance with facility, state, and federal regulations.
  • Participate in team meetings, case conferences, and in-service training to enhance professional development and stay current with best practices in physical therapy.
  • Ensure a safe and supportive environment for all residents, adhering to facility policies and procedures.
