About The Position

The Case Manager is responsible for planning and coordinating services, providing case management for a caseload of 25 clients, completing CAFAS Assessment and developing Individual and/or Family Service Plans, reviewing client progress, and determining case outcomes to assist clients in achieving their goals. In addition, the position facilitates youth prevention groups and conducts community outreach efforts to increase client referrals.

Examples of Work
  • Maintain a caseload of 25 clients.
  • Provide case management services to children, youth, and their families referred through the Department of Juvenile Services (DJS) and other child‑serving agencies.
  • Coordinate and provide services that are safe, timely, effective, efficient, equitable, and client‑centered.
  • Initiate contact with the family within 48 business hours of receipt of the referral to schedule an initial home visit and explain the purpose of the program; meet with the parent/guardian and youth within 10 business days.
  • Complete an intake assessment using the CAFAS as part of the face‑to‑face meeting.
  • Utilize the assessment process to continually measure youth progress and provide outcome data on the extent to which family functioning improved over the course of service provision.
  • Provide crisis intervention; address parent/child conflict; teach pro‑social skills; improve relationships between the family, community, and school.
  • Develop and maintain relationships with referring agencies staff and community organizations (i.e., school system, Pupil Personnel Workers, Inter‑Agency Council, etc.).
  • Identify and refer to appropriate, individualized services aimed at increasing protective factors and decreasing risk factors associated with delinquency‑related behavior.
  • Serve in the Community In‑service Network (CINS) program – refer clients to individual and family counseling, group counseling sessions, parenting classes, mentoring, tutoring, crisis intervention, problem‑solving, conflict resolution, anger management, home visit, life skills planning, and other specialized services as needed.
  • Provide required data and information regarding client assessment and progress in the CINS program.
  • Assist with the development, preparation, and submission of monthly and quarterly reports for funding sources.
  • Provide information regarding community resources and outreach efforts.
  • Participate in county‑wide community outreach activities/events.
Minimum Qualifications
  • Bachelor’s Degree from an accredited college or university in Business or Public Administration, Social Service, Sociology, Psychology, or a closely related field.
  • Relevant work experience which includes coordination of group activities, preferably with disadvantaged individuals.
  • Valid Class C driver’s license.

Any equivalent combination of relevant training, education, and experience may also be considered.

Preferred Qualifications
  • Master’s degree in the behavioral sciences, human services or social services fields.
  • 3 years of supervisory experience.
  • Child welfare experience and knowledge.
Conditions of Employment
  • Meet all training and performance standards and demonstrate proficiency as required by the agency.
  • Wear and use agency protective apparel and equipment in the performance of assigned duties, if applicable.
