Registered Nurse - Screening Care Coordinator

This position is located at The Martinsburg Veterans Affairs Medical Center in Martinsburg, West Virginia. The CLC Admission Coordinator Registered Nurse (RN) is an essential role within our organization, providing critical support in the admission, transition, and care of Veterans within the CLC.

The coordinator's responsibilities in leadership, collaboration, assessment, and data management are indispensable for maintaining high standards of care and optimizing resource utilization in the CLC. The coordinator is a subject matter expert in eligibility, co-payment, 10-10EZ & 10-10

EZR updates, means testing and coordinating with Acute Care services for Patient Flow/Difficult Discharge and care planning processes.

The position's responsibilities are foundational to maintaining high standards of care, fostering collaboration, and ensuring efficient and effective processes that are integral to the mission of our healthcare system

Pay:
Competitive salary, regular salary increases, potential for performance awards
