• Provide high-quality, patient-centered care to clients in their homes, focusing on their physical, emotional, and social well-being
• Conduct initial and ongoing assessments of clients' health status, develop and implement personalized care plans, and make recommendations to improve their quality of life
• Administer medications, treatments, and therapies as prescribed by physicians and other healthcare professionals
• Monitor and report changes in clients' conditions to healthcare providers, family members, and other stakeholders
• Educate clients and their caregivers on self-care techniques, disease management, and wellness strategies
• Collaborate with other healthcare professionals, including physicians, therapists, and social workers, to ensure comprehensive care

• Associate's degree in Nursing (ASN) or higher
• Minimum of 2 years of experience as a Registered Nurse
• Active license to practice as a Registered Nurse in the state of West Virginia
• Ability to work independently and as part of a team
• Excellent communication and interpersonal skills
• Compassionate and empathetic approach to client care
