All positions require on‑site patient visits.

• Providing hands‑on patient care within RN license scope (e.g., PICC lines, wound vacs, feeding pumps, NG and J tubes, drains, trach care, blood draws, ostomy care, IV antibiotics/TPN).
• Direct patient care as defined in the state Nurse Practice Act.
• Implementing the plan of care initiated by the Case Management RN.
• Accurate, timely documentation within 24–48 hrs.
• Educating patients and families about diagnoses and care plans.
• Assisting in skilled procedures, equipment preparation, and patient self‑care training.
• Coordinating home health services and reporting needs to other disciplines.
• Using equipment and supplies efficiently.
• Participating in professional development and on‑call duties as required.
• Maintaining patient privacy and professional standards.

• 1 year of recent nursing experience (waived for internal or new‑grad).
• Preferred: 1 year of home health experience and high‑acuity patient experience.
• Graduate of an accredited nursing school (Bachelor’s degree).
• Current California RN license and BLS certification.
• Valid California driver’s license with acceptable record.
• Current certifications and licenses sustained during employment.
• Self‑directed, critical thinker, strong interpersonal and communication skills, computer literate, basic math skills.

Compensation
:
Base pay $130,000–$160,000 per year; starting pay based on experience. Pay‑per‑visit model. Includes health benefits and a 401(k) plan with up to a 4% match.
