Responsibilities- Provide nursing care and support to dialysis patients in an outpatient setting.
- Collaborate with the healthcare team to develop and implement individualized treatment plans.
- Monitor patient status, administer medications, and perform procedures as authorized.
- Deliver education and support to patients and families regarding renal disease and self‑care.
- Maintain accurate and timely documentation in electronic medical records.
- Participate in quality improvement initiatives and clinical goal setting to meet CMS standards.
- Current LPN/LVN license in the state of practice.
- Current CPR certification.
- IV certification (required by state regulations).
- High‑school diploma or equivalent.
- Completed training program approved by the medical director and governing body, as required by CMS guidelines.
- BONENT or CHT certification preferred or as required by state regulation.
- Minimum of one year of experience in a hospital or clinical setting preferred.
- Basic computer skills and proficiency in MS Word and Outlook required; functional proficiency in DaVita computer systems within 90 days of hire required.
