Responsibilities

The Physical Therapist (PT, DPT, RPT) in Home Health assesses and evaluates patient care needs related to mobility, endurance, pain, and other medical conditions. Based on this assessment, the therapist develops a treatment plan, performs interventions to improve patient well-being, and evaluates progress.

Responsibilities include:

• Evaluating patients' functional status and therapy needs
• Assisting physicians in developing the plan of care
• Monitoring and reporting patient responses and condition changes
• Treating patients to relieve pain, restore function, and maintain maximum functioning according to standards
• Educating patients, families, and healthcare team members about the care plan and progress

Education and Experience

Licensure Requirements

• Current Physical Therapy license in Michigan
• Current CPR certification
• Valid driver's license, vehicle insurance, and access to a dependable vehicle
