We are seeking a Population Health Nurse to help coordinate clinical quality improvement and population health initiatives of all service areas, providing guidance in population health, workflow support, and all other clinical quality-related activities. This position will require weekly travel to other UGL clinics, but the primary work office will be in Houghton or Marquette County.
Responsibilities- Works alongside clinical staff to review and improve current clinical workflows, conduct performance assessments, and recommend/implement changes and improvements.
- Coordinates and executes care gap outreach.
- Assists clinical nursing staff with outstanding tests and referrals.
- Current MI licensure as an LPN or RN required.
- Bachelor’s degree in a healthcare-related field or 5 years’ experience in a primary care or healthcare quality setting preferred.
- Experience with PCMH preferred.
