The Licensed Practical Nurse (LPN) Patient Aligned Care Team is located at the Burlington Community Based Outpatient Clinic and performs all duties within the scope of their license including medication administration. Completed work is under the general supervision of an RN or MD/DO. Technically proficient in initiating, performing and completing assigned duties in providing care to variable patient populations. Completed work requires only a general review by a RN or MD/DO for appropriateness and conformity with established policies and procedures.

Duties and responsibilities include, but are not limited to the following:

* Knowledge and ability to appropriately carry out assigned patient care based on the patients' conditions; to use judgment in selecting the appropriate order and sequence of procedures and treatments and to accurately recognize, report and record relevant patient information.

* Works with the RN and/or MD/DO to appropriately orient/train less experienced LPN/LNs, Nursing Assistants and Health Technicians in relation to acceptable standards of practice in promoting optimal patient care delivery. Serves as a preceptor to support training of less experienced LPN/LVNS, CNAs, and HTs related to support duties for complex specialized tests/procedures.

* Ability to observe, identify and respond to the patient's needs for care, medication, equipment assisted care and patient/family education. In organizing and delivering care the LPN/LVN recognizes and considers emotional, cultural, spiritual, socio-economic, and age-related factors.

* Establishes constructive relationships with individual patients and their families to elicit feelings and attitudes, and to promote positive relationships, communication, and socialization skills. Fosters an environment of respect for individual patient and family rights to privacy and dignity in all aspects of care delivery. Effectively incorporates knowledge and understanding of established customer service standards in all interactions with patients, family members, and/or other internal/external customers.

* The LPN/LVN practices within the scope and licensure of a LPN/LVN in the nursing unit, clinic, or area of assignment, and demonstrates consistent practice when temporarily assigned to related nursing units and clinics.

* Performs support duties for complex diagnostic tests and/or specialized practices or procedures, which include preparing the patient, assisting in the diagnostic examination, preparing and handling specialized instruments or other specialized equipment, and monitoring the patient's condition before, during, and following the procedure.

* The LPN/LVN provides care of patients/residents and their environment contributing to optimizing patient/resident outcomes as a member of the multi-disciplinary healthcare team.

* The LPN/LVN maintains licensure, BCLS, and currency in required education and training as identified by regulating agencies, the PVAMC, VISN and National Agency.

* The LPN/LVN maintains patient/resident safety standards assuring patient /resident privacy, patient/resident physical safety, and patient/resident psychological safety.

* The LPN/LVN administers medications within the scope of their license consistent with MCM 11-03 Administration of Medications. Demonstrates the knowledge and skill to prepare, administer, and appropriately document actions taken specific to commonly prescribed oral, topical, subcutaneous, intramuscular and/or intravenous medication. Observation and documentation include the patient's response to medication administered and any noted change is reported to the RN or MD/DO.
