Title: Physical Therapist – Home Health
Location: Marion, OH (home health territory up to a 60-mile radius from the office)
Duration: 13 weeks
Shift: Day shift, 5x8 hours (08:00 – 17:00)

Job Responsibilities and Requirements:

The Physical Therapist will provide home health physical therapy services primarily to a geriatric population. Responsibilities include completing start-of-care and follow-up visits, developing and implementing individualized treatment plans, performing evaluations and reassessments, and delivering skilled therapy interventions in patient homes. The therapist is responsible for timely and accurate documentation, effective communication with the interdisciplinary care team, and maintaining productivity expectations of approximately 6–7 patients per day.

Travel within the assigned territory is required.

Required Experience & Qualifications:

• Minimum of 1 year of recent home health physical therapy experience

• OASIS experience required (including SOC visits)

• Proficiency with Home Care Home Base (HCHB) EMR required

• BLS certification required

• Active state PT license in hand

• Previous charge experience required

• Timely communication and documentation required

• Ability to work independently in a home health setting

• First-time travelers not accepted
