In this role, you will:

• Provide comprehensive case management services to patients in a home health setting
• Collaborate with the interdisciplinary team to develop and implement individualized care plans
• Ensure compliance with all state and federal regulations
• Document patient care and progress accurately in the EPIC EMR system
• Participate in rotating weeknight on-call duties from

Requirements:

• Current Iowa RN license
• BLS (AHA) certification
• Minimum of 2 years of Case Management experience
• At least 1 completed travel contract
• Ability to float within the same discipline across local campuses
