Position: Registered Nurse - Home Telehealth

Overview

Summary The Staff Registered Nurse provides direct and indirect nursing care to patients and families in the Home Telehealth program.

Responsibilities
  • The Staff Registered Nurse provides direct and indirect nursing care to patients and families in the Home Telehealth program
  • He/she provides for the promotion and maintenance of health and prevention of illness and injury based upon the nursing process which includes systematic data gathering - assessment - appropriate nursing judgment - and evaluation of human responses to actual or potential health problems through such services as case finding - health teaching - health counseling
  • Provision of care supportive to or restorative of life and well-being
  • Executing medical regimens including administering medications and treatments prescribed by a licensed or otherwise legally authorized provider
  • Major duties include but are not limited to:
    Demonstrates clinical skills and knowledge in therapeutic procedures which relate to home care
  • Effectively utilizes support services - education - and other resources and takes action through referrals - consults - and direct actions to ensure patient care needs are met
  • Identifies and resolves routine problems related to patient care coordination - promoting efficient utilization of resources and cost-effective care
  • Accurately interprets policy to patients - families or visitors and refers them to appropriate personnel for problem resolution or to enhance CCHT services
  • Assess the need for special medical equipment and/or supplies and arranges for procurement
  • Participates in in-service educational programs regarding CCHT and discharge planning for inpatient and ambulatory care professional staff as needed
  • Demonstrates clinical competence in completion of care and practice specific procedures/competencies according to the Competency checklist for CCHT
  • Assesses learning needs of patients/families and assists them to acquire new skills
  • Assists families - and other staff members with developing coping mechanisms to provide care in the home setting
  • Assists families and other staff members with developing coping mechanisms in emergency situations
  • Exhibits broad knowledge base of home telehealth/medical/surgical/psychiatric nursing
  • Correctly completes encounters
  • Understands and appropriately performs telephone triage/management
  • Makes appropriate referrals and collaborates with community agencies to provide services to patient and families
  • Collaborates with the interdisciplinary team members - health care personnel in primary care clinics - ambulatory care and acute hospital units - Veterans Community Living Centers and community agencies
  • Collaborates with community health agencies concerning outpatient health care issues and resources
Qualifications

Basic Requirements:

  • English Language Proficiency
  • In accordance with 38 U.S.C. 7403(f) - no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English
  • Graduate of a school of professional nursing approved by the appropriate accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant:
    The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE)
  • OR Individuals attending a master's level bridge program in nursing who have completed coursework equivalent to a bachelor’s level degree in Nursing may have opportunity to become registered as a nurse with a state licensing board prior to completion of the bridge program
  • Upon achievement of a State license - the individual may be appointed on temporary basis and later converted to a permanent appointment upon successful completion and graduation from the bridge program

  • OR In cases of graduates of foreign schools of professional nursing - possession of a current - full - active - and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing to warrant an appointment as a Nurse who has completed an associated degree/entry level Nursing education program
