• Certification:

  • Must be Board Certified or Truly Board Eligible

  • Proficiency required in epidurals, spinals, and blocks

• Licensure:

  • Active Indiana license preferred

  • Will consider those needing to obtain one

  • IN PCF (Patient Compensation Fund) coverage required

• Day Hours: 7:00 AM - 4:00 PM

• Call Hours: 4:00 PM - 7:00 AM

• Call Type: OR & OB, Primary and Secondary

• Call Response Time: 20 minutes

• Supervision: By MDs

• Number of Providers Per Day: 6
