• Provide care coordination and support for High Fidelity Wraparound or Intensive Care Coordination for clients
• Conduct in-person, community-based child and family team meetings and support crisis services
• Deliver ongoing case management services, including assessment, planning, linking, monitoring, referral, and advocacy
• Assist clients in building a natural support team and integrating community resources into service delivery
• Provide community education as assigned and report critical incidents as per company policy

• Bachelor’s Degree in a human services-related field, or an Associate’s Degree with at least three years of relevant experience, or a High School diploma with a minimum of five years of supervised experience in behavioral health
• Prior experience in mental health services as detailed above
• Ability to effectively communicate through written, verbal, and virtual methods
• Skills in customer service and problem-solving
• Knowledge of various community resources and dynamics of assigned disability groups

• Experience in behavioral health care coordination or case management
• Familiarity with community-based services and support systems
• Additional certifications relevant to mental health and behavioral health services
