The RN Home Health is responsible for providing an initial, resumption, and recertification evaluation of the Home Health patient in their home setting, along with other routine home health visits. This includes age appropriate interventions that meet physical, emotional, developmental, psychosocial, and educational needs. The RN Home Health is responsible for the coordination of initial disciplines to begin care, including the planning of care throughout the patient episode, collaborating with the physician, patient, family member and team manager to develop the expected patient visits, scheduling the visits to the appropriate team member, incorporating therapeutic, preventative and rehabilitative nursing actions.

The RN Home Health communicates with all team members including RNs, LPNs, Therapists, Social Workers, Home Health Aides, the Physician, patient, family, and other internal or external individuals to ensure that the patient's plan of care is appropriately developed.

Shift: Mon-Fri 8:30a-5pm 40hrs 6 patient points per day Holiday: yes regular shift

Epic Remote or Home Care Home Base Systems;
Wound Vac's;
Wound Care; IV Management;
Assess and Teach disease process ie CHF; COPD etc;
Catheter insertion and care; PICC line dressing changes.

Scrubs-no jean

RN - Reg Nurse (Single State) or RN-COMPACT - RN - Multi-state Compact

BLS - Basic Life Support or BLS-I - Basic Life Support - Instructor

GADL - Georgia Driver's License or VSDL - Valid State Driver's License

PVI - Private Vehicle insurance

Minimum 1 year Medical/Surgical Nursing Required orHome Health, and ICU experience Preferred orMust possess extremely strong critical care nursing skills.
