Base pay: $44,600 – $61,400 per year. Pay per visit: $28.00 – $40.00. Full-time hours: 40 per week.

• Provide professional nursing services under the direction of an RN in compliance with state regulations and Center Well policies.
• Deliver skilled nursing interventions for patient care, rehabilitation, and preventative services.
• Apply clinical practice standards to achieve individualized patient goals.
• Consult with RN supervisor regarding treatment plan adjustments and accept oral orders from physicians as permitted.
• Maintain systematic, goal-driven implementation of the care plan.
• Coordinate with physicians, managers, and team members to address patient needs and plan discharge as appropriate.
• Uphold confidentiality and share information only when authorized.
• Ensure availability of supplies and equipment required for safe care.
• Document care activities and submit required reports.

• Graduate from an accredited Licensed Practical Nursing program or School of Vocational Nursing.
• Current LPN license in the state of practice.
• Valid driver's license, auto insurance, and reliable transportation.
• Current CPR certification.
• At least two years of clinical experience as an LPN/LVN, preferably in home health or hospice.
