Responsibilities
  • Provide one-on-one nursing care to patients in their homes.
  • Develop and follow individualized care plans for each patient.
  • Collaborate with the care team to achieve the patient’s goals.
  • Utilize home health software and other computer-based learning modules as required.
  • Communicate effectively with patients and families; document care accurately.
Qualifications
  • Graduate of an approved school of professional nursing, Required.
  • Strong critical care assessment and clinical skills.
  • Strong communication (written and verbal) skills.
  • Ability to write and follow directions to the patient's homes.
  • Ability to use home health patient software, email, internet, and computer-based learning modules.
  • Good understanding of home health operations.
  • Current CPR/BLS.
  • Must be able to travel to patients' homes and have reliable transportation, valid driver's license, and pass MVR check.
  • Positive attitude and strong work ethic.
