Job SummaryCore Health Provider Services, Inc has an employment opportunity as a Developmental Disabilities Professional for Residential Services at our office in Marietta, GA. Our mission is to serve adults with special needs and elderly clients, by providing specialized services activities, learning opportunities and supports to help individuals with intellectual and/or developmental disabilities live independently in their communities and Group homes. Supports are individually planned and tailored to meet the specific needs of the individuals we serve.

- Must be 21 years of age or older
- Must have Bachelor’s degree in social services or related field
- Must have management experience and be able to manage staff
- Must be detail-oriented and able to maintain confidentiality
- Computer systems – Therap, Health Risk Screening Tool, IDD Connects
- Intimate knowledge of DD Services and funding sources (NOW/COMP Waivers, state funded) and service rates
- Budgeting and maximizing service units of individuals in all services
- Must have effective communication skills and be able to speak, listen, write clearly
- Must submit to a DBHDD criminal background check
- Must provide pre-employment physical and TB screening
- Must be able to work in the U.S.
- Must have or able to obtain CPR/First Aid certification
- Must be a positive team player and maintain a positive attitude
- Must be flexible in working hours and be willing to work additional hours as needed
- Must be able to work with different cultural and socioeconomic backgrounds
- Must have at least 3 years of experience as a Direct Support Professional or related work experience with persons with developmental disabilities.
- Document monthly home visits
- Daily review of progress notes
- Monthly documentation (summaries, HRST updates, DDP ISP review notes)
- Ensure that the services, supports, treatment, and care respect the vision of the individual
- Communicate effectively with staff, and parents/providers
- Completes incident reports (non-critical, critical, death reports and follow-ups)
- Focus on Client Needs:
Anticipate, understand and respond to the needs of internal and external clients to meet or exceed their expectations within the organizational parameters - Foster teamwork:
Work cooperatively and effectively with others to set goals, resolve problems and make decisions that enhance organizational effectiveness - Oversee the services and supports provided to participants that include;
- Monitors and/or participates in the implementation and delivery of the Individual Service Plan (ISP).
- Supervises the delivery of service and ensures the strategies reflect the goals and objectives of the ISP.
- Providing, arranging, or overseeing curricula used in staff training and directed to service delivery in the context of the individual’s goals and objectives;
- Participates in the development of the ISP
- Scheduling
- Conducts functional assessments to support formulation of the individual's plan for delivery of all waiver services
- Creates outcome measurement and quarterly summaries on assigned individuals
- Involved in identifying ongoing supports as needed (medical and/or behavioral) in collaboration with appropriate personnel
- Complete the SIS, HRST, and ISP trainings by DBHDD. In addition to any others as needed or required, if qualified.
- Supervising high intensity services that address health and safety risks for the individuals that include:
Reviewing and the implementation and effectiveness of the Behavior Support Plan or Crisis Plan. Track, revise, and ensure training of target behaviors and replacement behaviors to all staff - Communicates with providers and service monitors
- Meets assigned responsibilities utilizing Performance Indicators.
- Active participation in the planning meeting documented in either the meeting minutes/notes and/or progress notes prior to ISP meeting.
- Documents contact with the SC prior to the ISP date.
- Other duties as assigned
