Position Summary:

As an Independent Optometrist leasing space inside Sam's Club stores, you will take a leading role in patient care for eye and vision health as well as general health and wellbeing. You will have the autonomy to set your own schedule, fee structure, and to maintain full control over patient flow.

• Provide comprehensive eye exams and vision care services to Sam's Club customers.
• Manage patient schedules and maintain professional standards of care.
• Set and collect fees, retaining 100% of exam/professional service revenue.
• Operate independently while complying with state regulations and Sam's Club lease terms.

• Choose your days and hours of practice.
• Control your patient schedule.
• Set your fee schedule.
• Opt into advanced equipment/instrumentation.
• Receive administrative and/or opt‑tech support services, where state law permits.

• Doctor of Optometry (OD) from an accredited optometry school.
• Valid state optometry license.
• General Liability Certificate and Professional Liability Insurance Certificate.

• Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) in ophthalmology, with a valid state ophthalmology license.
