Job summary

The post holder will be an expert clinical practitioner able to demonstrate specialist clinical competence and knowledge base. With the support of the Lead Nurse, the post holder will lead the Tissue Viability service at the designated work base, whilst providing team support at other sites as required.

Main duties of the job

To provide specialist clinical advice and psychological support to patients with complex wounds, liaising with and referring on to other healthcare professionals as required. The post holder will provide Tissue Viability and Pressure Ulcer prevention education and training to nursing and clinical staff across the site, including supporting the wider multidisciplinary teams with audit and evaluation. The post holder will support the development and implementation of the Trust Tissue Viability Strategy and support the development of systems, guidelines and policies that are evidence based.

The post holder will deputise for the Lead Nurse for Tissue Viability, as required.

Job responsibilities
  • Manage time effectively and identify priorities of care delivery and Tissue Viability caseload.
  • Maintain standards of the Tissue Viability Service; led by defined guidelines, protocols, procedures, codes of conduct and followed at own discretion.
  • Respond and escalating to changes in the needs and/or environment. Support in the planning to ensure effective use of staff and resources.
  • Refer to and work with multidisciplinary teams to develop and support the service user, family and carers; providing specialised advice on complex wounds and pressure prevention to facilitate the delivery of individualised care.
  • Use numeracy, literacy, digital photography and technological skills to support safe and effective delivery of care.
  • Carry out required Tissue Viability nursing skills with dexterity and accuracy.
  • Carry out systematic and evidence-based assessments to give holistic care, considering physical, social, cultural, psychological, spiritual, genetic and environmental factors that may influence service users pathway or treatment.
  • Use interprofessional and interpersonal skills to facilitate the best communication of highly complex, highly sensitive and highly contentious information to patients, families/carers, members of the public and workforce.
  • Accept accountability and responsibility for delivery of care and decisions within clinical practice, ensuring all care is delivered within scope of practice.
  • Maintain accurate, timely and legible records of documentation and care delivered, including how consent is obtained and whether the patient has capacity.
  • Provide and support staff to deliver safe wound care. Utilize clinical skills such as conservative sharp debridement, complex negative pressure wound therapy dressings and compression therapy to meet complex care needs while adhering to guidelines, policy and codes of conduct.
  • Analyze, assess, plan, implement and evaluate Tissue Viability care needs continuously, empowering the service user to achieve optimal health through developing and providing highly specialist packages of care and advice.
  • Advocate for patients' best interests and respect their moral values, beliefs, dignity and ethics.
  • Analyze and interpret complex situations; mitigate risks and/or issues to patient/client clinical need; escalating to senior team when required.
  • Support the Trust to manage redeployment to ensure safe patient care and skill mix when identified by senior leadership teams.
  • Safeguard vulnerable adults, young people and children, including patients' valuables, in accordance with Trust policies and protocols.
  • Work with medical equipment library to ensure correct use of pressure-relieving equipment and that it is in good working order and within service calibration dates; elevate any damages and required maintenance. Influence purchase or supply decisions for pressure-relieving equipment, making recommendations to appropriate suppliers.
  • Ensure data and information of the service user is treated with confidentiality, sensitivity and adheres to the Data Protection Act 2018, GDPR and Caldicott Guidelines.
