Job Description

The Registered Nurse, RN, is a member of the interdisciplinary team who works together to meet the physical, medical, psychosocial, emotional, and spiritual needs of hospice patients and families facing terminal illness and bereavement. The RN provides coordination of care, ensures continuous assessment of each patient's and family's needs, and implements the interdisciplinary plan of care.

Primary Function

The RN collaborates with the interdisciplinary team to deliver comprehensive hospice care, including assessments, education, supervision, and participation in quality improvement activities.

Job Responsibilities

Case management and supervision of nursing staff and volunteers.

Follow VITAS policies and standards, practicing within the scope of the RN license.

Perform initial and ongoing assessments, including physical, psychosocial, spiritual, and bereavement evaluations.

Identify and utilize resources and referrals such as PT, OT, dietary, pharmacy, and medical equipment.

Document care activities accurately and timely.

Educate patients, families, and caregivers on care plans and pain management.

Participate in patient care and team meetings, providing shift reports and effective communication.

Participate in bereavement support and on-call rotations as needed.

Develop, coordinate, and follow the individualized care plan in collaboration with the IDG.

Obtain physician orders when necessary and delegate tasks appropriately.

Ensure compliance with infection control practices and quality improvement initiatives.

Maintain professional development through training, conferences, and orientation of new staff.

Serve as a patient and family advocate, handling complaints and conflicts professionally.

Job Requirements

Current RN license in the state of practice.
Minimum of two years of nursing experience in hospice, home health, or community health within the last five years.
Reliable transportation and a telephone (cell or land line).

Physical Demands

Must be able to assist in lifting patients, stand or walk for extended periods, and perform physical tasks with or without reasonable accommodations.

Qualifications

Licensed to practice nursing in the state where the program is located.
Experience in hospice, home health, or community health preferred.
