Position: Physical Therapist Home Health Sign-On Bonus

Physical Therapist Home Health Sign-On Bonus

Join Pinnacle Home Care as a Physical Therapist with a $5,000 sign‑on bonus for full‑time positions. Service Area:
South Naples. Salary Range: $113,000 – $120,000 per year.

Key Responsibilities
  • Perform initial and ongoing patient assessments, including OASIS evaluation and documentation.
  • Develop and implement tailored treatment plans in consultation with physicians and clinicians, adjusting as needed.
  • Collaborate with the interdisciplinary care team to optimize patient outcomes.
  • Provide education and support to patients and families/caregivers on therapy programs.
  • Maintain accurate clinical records and report any changes in the patient’s condition to the physician.
  • Supervise Physical Therapy Assistants in accordance with agency policies and state regulations.
  • Participate in in‑services, case conferences, and meetings to promote coordinated care with healthcare team members.
  • Identify patient and family/caregiver needs for additional services and make appropriate referrals.
  • Confirm daily visit schedules to ensure seamless coordination with other staff.
  • Communicate patient progress and changes with physicians and care teams.
  • Document all visits and progress notes on the same day of service.
Qualifications
  • Current PT license (state‑specific).
  • Graduate of an accredited physical therapy program.
  • Valid BLS certification.
  • Minimum one year of PT experience, home health experience preferred.
  • Excellent communication, organizational, and time‑management skills.
  • Valid driver’s license and access to a reliable and insured vehicle.
  • Ability to visit homes that may not be wheelchair accessible.
  • Proficient hearing, eyesight, and dexterity for physical assessments and demonstrations.
  • Engage in physical activities such as walking, standing, stooping, and occasional lifting or turning of patients.
  • Proficiency with electronic medical records is a plus.
